TerminalCompletionSourceFailureHandler

Receives original non-cancellation failures from merged completion sources. Ordinary exceptions are isolated; unexpected errors are reported before they fail collection and cancel sibling sources.

Implementations must be thread-safe, return promptly, and must not throw. Different source child coroutines may report concurrently while the merged engine collects the remaining sources.
